Your Specific Responsibilities:
This individual will be responsible for all supervisory duties including, but not limited to, hiring and selection, developing performance plans, conducting performance appraisals, planning, and directing their work, approving leave, and taking disciplinary action as appropriate.
The incumbent is part of a global public health organization and is expected to support the Florida Department of Health in Monroe County in its mission to protect, promote and improve the health of all people in Florida through integrated state, county and community efforts and its vision to be the Healthiest State in the Nation. Furthermore, the incumbent is expected to incorporate the agency values of Innovation, Collaboration, Accountability, Responsiveness and Excellence in all aspects of their work.
This position is based out of the Gato Building in Key West and will require travel to other DOH Clinical sites in Monroe County based on agency need. The incumbent will evaluate clients in a variety of settings within the scope of Florida State and Federal guidelines, and Florida Department of Health in Monroe County policies. Services will be performed and documented accurately utilizing designated electronic medical records and established policies and procedures. The incumbent will follow Florida Department of Health in Monroe County policies and procedures, including standing orders.
Responsibilities for this position includes supervision of the Epidemiology, STD, Hepatitis and TB programs and staff. This position is required to maintain confidential information in accordance with the DOH and Monroe County information security, policy, protocols and procedures. Confidential information data sets: clinical data (HMS), immunization data (FLShots), HIV/TB, Social Security data, HIV/AIDS case reporting and surveillance, PRISM, ADAP, CareWare, Pharmacy Forms System, HMS report portal, Provide Enterprise for FL ADAP.
EPIDEMIOLOGY:
- Develops program structures and needs
- Responsible for all budget managing related to programs that this position manages
- Implements new policies, standing orders, policies/procedures, and best practices within the Program.
- Coordinates disease investigation operations
- Annually creates a balanced budget and ensures spending is allocated appropriately and within program guidelines
- Manages, collects, and reports findings of communicable disease, including disease surveillance and case management of communicable reportable diseases in Monroe County as required by the State and County Health Department
- Conducts disease surveillance and reporting, case investigations, outbreak investigations, implementation of control measures, interrupt disease transmission or mitigate adverse health impacts, and planned studies of important public health problems within the county
- Provides leadership, direction, and epidemiological capacity during public health investigations, and is involved in forming partnerships to increase the capacity of the health department to monitor, improve, and maintain community health
- Coordinates medical biohazard wastes for the Gato facility
STD/TB/Hepatitis:
- Primary contact for Monroe CHD epidemiology surveillance and state communicable disease reporting
- Coordinates surveillance activities and responsible for management and supervision of the Monroe CHD communicable disease data collection, analysis and reporting
- Works with Environmental Health staff to promote and protect the health of residents and visitors. Develops and implements communicable disease prevention activities
- Conducts Internet Partner Services using the Internet and select mobile dating applications to notify persons exposed to, or at-risk for STDs, including HIV. All partner notification activities will be carried out within established communication parameters as outlined in the Internal Operating Policy (IOP) 360-19-17, Sexually Transmitted Diseases (STD) and HIV Partner Notification via Mobile Dating Applications
- Coordinates and provides testing to at risk population
- Collaborates with internal DOH programs and community programs to offer and to ensure high risk/at risk population is offered testing and receive appropriate follow up care and/or referrals to resources as needed
- Coordinates with all clinic sites/professionals that information is being used accurately and testing is appropriate along with ensuring follow up
- Cooperates and coordinates with all staff to facilitate case management and cases to be seen in clinic/field as needed and assure maximum productivity. This includes collaboration with physicians, practitioners, laboratory, nursing, and paraprofessional staff
- Responsible for collection and reporting of laboratory data using State of Florida and CDC Guidelines
CLINICAL:
- Utilizes skills of interview, observation and evaluation to determine the medical, social and psychological needs of client/family/community and provides interventions as needed
- Counsels clients/family/community in basic health promotion and risk behaviors, including but not limited to: communicable disease control, maternal and child health, family planning, nutrition, growth and development, safety and anticipatory guidance
- Advises and counsels client on health promotion, STD & HIV risk reduction, and care and management of disease
- Provides case management to established or referred clients in need of nursing intervention, makes appropriate referrals, and then evaluates the effectiveness of this intervention
- Demonstrates willingness to cross train and increase competency in clinics as needed
- Assists examiners as needed
- Assists in review of electronic laboratory results and provides follow-up and case management following Florida Department of Health in Monroe Local Policy
GENERAL:
- Responds promptly to emergency situations and informs supervisors of significant or critical issues that may affect the division or department
- Complies with HIPAA guidelines Performs duties as described in the Florida Department of Health's Information Security policies, Protocols and Procedures, ensuring client information is secure 100% of the time
- Completes EARS, Timesheets, and People First Entries within mandatory time frames
- Schedules time off in advance and collaborates with nursing supervisors to ensure work responsibilities are covered when absent
- Complies with SMART expectations related to position
- Communicates, collaborates, and assists other professionals and paraprofessionals in other units within the agency (Examples: health fairs, immunization outreaches, disaster support)
- Attends and participates in quality improvement activities, including medical record review, in-service programs, and staff meetings
- Obtains all Florida Department of Health in Monroe County and Florida Department of Health as required yearly training
- Provides outreach activities to provide services and education to populations that may include rehabilitation centers, sites for the homeless, incarcerated individuals, and others identified as at risk
- Provides educational needs for the community, health care providers and residents of the county concerning communicable disease prevention and control
Other duties as assigned:
- Assists as needed in all Florida Department of Health - Monroe County clinical settings based on agency need
Required Knowledge, Skills, and Abilities:
- Knowledge of Epidemiologic principles and practices related to public health surveillance, case investigation, outbreak investigation, and disease control.
- Knowledge of microbiology sufficient to be able to work with public health and clinical laboratories.
- Knowledge on the utilization of statistical analysis software for data analysis.
- Knowledge of Microsoft office, Excel, Word.
- Ability to plan and prioritize multiple work assignments.
- Ability to communicate clearly and concisely in writing and orally to diverse audiences including the general public, public health leaders, and health care providers.
- Knowledge of professional nursing and public health principles, practices, and techniques in specialized areas.
- Knowledge of the theories and concepts of preventive health care
- Knowledge of human anatomy and physiology
- Knowledge of community resources
