Arisa Health is seeking candidates to fill a Care Coordinator position in our Russellville Certified Community Behavioral Health Clinic. The Care Coordinator is a member of the health care team who helps individuals overcome possible barriers to care. The Care Coordinator works in collaboration with clients, multidisciplinary care providers and community partners to coordinate treatment and services. The person who fills this position will provide timely intervention and follow up to individuals experiencing a behavioral health crisis. They will develop care initiatives to assess and coordinate care management for clients and serve as a client advocate, and will assist in the identification and improvement of service delivery.A bachelor's degree is required. Candidates must be proficient with computers including the ability to type, and familiarity with word processing and e-mail programs; and must have good phone etiquette and excellent written and verbal communication skills; the ability to handle multiple tasks and work independently with minimum supervision; familiarity with the behavioral and primary health care system(s); knowledge of local community resources; and the ability to establish and maintain effective working relationships with others.
Work hours: Full time, Monday - Friday, 8:00 a.m. - 5:00 p.m. No phone calls please.
