Granite Bay Care, Inc. (GBC) is a multi-state human service agency providing intensive clinical, nursing, residential, and community-based support services to adults with cognitive, intellectual, mental health, and physical disabilities.

We are currently seeking a full-time Case Manager to work with our clients and their families to elicit services as needed, promoting choices in employment, living and social decisions.

Responsibilities

  • Maintain a case load of twenty-five individuals, adhering to all standards set forth in the Case Management Manual. (in the event of a case manager vacancy, be able to maintain services to all consumers served through an increased caseload through temporary reassignment of cases)
  • Develop and maintain consumer and family relationships which allow for necessary observation, compilation of history and assessment of needs.
  • Maintain the rights of consumers served and advocate for the needs of the consumer to include financial resources.
  • Ensure that his/her own case management activities are directly linked to a consumer driven needs based model with the Person Centered Plan acting as the primary vehicle to this model.
  • Create and monitor Person Centered Plans and ISPs with adherence to accepted treatment principles, practices, and techniques.
  • Promote health and safety in the services provided to all consumers and follow outlined Quality Management processes.
  • Serve as a representative payee for individuals receiving SSI payments in order to manage and administer consumer finances.
  • Interpret and apply Federal and State Regulations governing programs such as Social Security Disability and Medicaid Waiver programs in order to provide services.
  • Plan testimony and testify at legal hearings regarding guardianship.
  • Complete all documentation as required to provide high quality services, complete billing, and maintain waiver classification.
  • Adhere to all standards set forth in the Case Management Manual.
  • Maintain APS and EIS Certifications.
  • Attend conferences, workshops, and seminars to improve professional skills to benefit the agency, consumers, and staff.
  • Represent GBC in the community, serving as a member on relevant committees, task forces, etc.
  • Perform all other duties as assigned.

Qualifications and Requirements

  • A four-year bachelor’s degree from an accredited institution with specialization in psychology, behavioral health, social work, special education, counseling, nursing or a closely related field; three years’ experience in one or more of the areas listed here.
  • Service Planning and/or Case Management experience required. Two years’ experience providing Case Management to individuals with ID.
  • Ability to make sound differential diagnostic and treatment decisions.
  • Ability to problem solve; practice time management and organizational skills.
  • Ability to work independently in an environment; requires the ability to be flexible and manage multiple priorities.
  • Ability to read, understand and explain procedures pertaining to trusts, court certification, guardianships and the consent decree.
  • Must have a valid Maine driver’s license.
  • Active automobile insurance policy

Full Time and Part Time Positions Available

Pay: $23.00 - $25.00 per hour
