Case Manager Substance Use Disorder Treatment Center
Job description
Sunrise Centre is a 41 bed Residential Treatment Center supporting individuals with substance use and co-occurring mental health disorders. Sunrise Centre also provides outpatient treatment services to this population and case management services that support both our inpatient and outpatient clients.
Sunrise Centre is looking for a full time Case Manager to provide case management and coordination of care services to SC clients. The Case Manager position requires a minimum of a Bachelor's Degree in Social Work or a Human Services field. The Case Manager will be required to hold a CADC credential from MCBAP or be willing to get on a development plan at the start of employment to pursue this credential.
Case management responsibilities include the following:
- Provide on-going support and case management expertise to persons in treatment for substance use disorders and navigating recovery,
- Work with clients to develop individualized case management plans,
- Support inpatient and outpatient clients in accessing resources, services, and supports in their community,
- Coordinate care with the client and other treatment providers,
- Help clients with resumes, applications for resources or jobs, and support clients in accessing the tools they need to address barriers in their lives,
- Maintain timely documentation of services provided in a medical record system,
- Update case management plans with clients as needed to ensure relevant support is provided,
- Maintain strict confidentiality of clients and the center, complying with HIPAA and 42-CFR confidentiality rules and regulations,
- Obtain releases of information as needed to support clients in coordinating and accessing the care they need,
- Collaborate with treatment team to support clients and support provision of high quality care in the center,
- Represent the agency in all communications with integrity and high ethical standards,
- Complete intake assessments as appropriate for individuals coming in to residential treatment,
- Engage in continuing education opportunities to build and strengthen expertise in working with the populations served,
- Facilitate case management supportive groups weekly to support skill building of residents,
- Participate in treatment team meetings, weekly supervision, and other tasks to support the agency as needed.
Job Type: Full-time
Pay: $17.00 - $22.00 per hour
