Job description
- The Clinical Marketing Liaison works with the multidisciplinary team to ensure that an efficient and informed transfer/discharge occurs across the care continuum. In collaboration with the RN Case Manager/RN Managers formulates a plan of care to meet the individual needs of the patient. The position will function as a liaison between the acute setting and sub-acute levels included, but not limited, to nursing home, hospice care and/or family caregiver. The Clinical Marketing Liaison provides education to the patient/family members on evidence-based geriatric best/practice to promote positive outcomes throughout the continuum for an effective transition. The liaison collects, inputs and maintains specific geriatric data necessary for the completion of specific dashboards which are utilized to enhance and coordinate the needs of the geriatric population. The Clinical Marketing Liaison is responsible to complete all NICHE GRN education and continue participating in geriatric nursing education programs to maintain knowledge base on annual bases. This position also evaluates the patient prior to them being accepted at the facility in accordance with current federal, state and local standards, guidelines and regulations, established policies and procedures and as may be directed by the Administrator.
- 5 years clinical experience including 1 year of case management + 4 years experience working with client relations marketing or account management
- LPN license in LA (Bachelor's degree substitutes for client/marketing experience; RN license qualifies in lieu of LPN)
- NICHE GRN Certification (required within six months) Certified Health Coach (required within six months)
- Ability to manage complex projects and work with deadlines. Interpersonal & human relations skills and oral & written communication skills

- Collaborates with members of the multidisciplinary team to identify appropriate patients for transition care services, utilizing established criteria to facilitate successful transition to all levels of care.
- Prioritizes referrals and activities according to intensity, need, and required follow-up. Performs referral mining activities and markets program to key stakeholders.
- Ability to interact with discharge planners in an effective and efficient manner. Ask for appropriate and adequate resident information
- Coaches and supports all patients in addressing critical issues. Evaluates the patient/family based on developmental needs and assessment of the geriatric population.
- Recognizes each of the following aspects of patient's condition: diagnoses, medications and support systems. Utilizes critical thinking skills in achieving successful outcomes related to disease/medication management. Responsible for assisting the case manager/social worker with the coordination of discharge planning including primary care/specialty follow-ups, clinic appointments and SBAR communication with referral agencies.
- Explain facility philosophy, policies and procedures to potential and new residents and resident's representatives. Consistently work cooperatively with residents, residents' representatives, facility staff, physicians, consultants and ancillary service providers. Supports and promotes patient self-management and empowers patients/families to achieve maximum levels of wellness and independence
- Communicates critical information related to risk issues or other need to know information with administration, risk management, medical management, medical staff and patient advocates.
- Participate in marketing the facility and maintaining an acceptable census. As a member of the multidisciplinary team, works to obtain resources for high risk patients for readmission related to inadequate family support, poor understanding and social restrictions. Notifies and discusses findings and concerns with attending physicians and provides possible suggestions for additional support, i.e. Geriatric Medical Clinic, Council of Aging, Elderly protection, etc.
- Quality Performance
- Monitors division performance in accordance with our four organizational priorities of patient and customer satisfaction, employee satisfaction, financial performance, and operational excellence. Serve on, participate in, and attend various committees of the facility as required, and as appointed by the Administrator.
- Assist in development, administering, and coordinating of department policies and procedures. Develop and implement policies and procedures for the admissions/marketing needs. Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Administrator
- Collects and maintains specific data necessary for the completion of specific NICHE dashboards utilized in the development of best practice guidelines in the care of the geriatric population.
- Participates in measuring clinical outcomes and data procurement. Represents the care transition program on performance improvement teams
- Assist managers in identifying problems and developing tools.
- Marketing and Sales
- Develop relationships with a growing base of referral sources, leading to a consistent flow of quality referrals to the faculties. Actively see out and identify new referral sources and maintain a current prioritized account list.
- Communicates, collaborates with community resources to meet specific geriatric needs and to enhance patient outcome. Maintains knowledge regarding program initiatives based on geriatric population/needs and incorporates the outcomes into practice.
- Participate in marketing the facility and maintaining an acceptable census.
- Effectively networks with current and past customers to solicit referrals and maintain an active involvement in the community organizations and associations
- Other duties assigned deemed as necessary
