  • This is a temporary position. The contract is scheduled to terminate November 30, 2023.

The primary service site for this position will be Children's Primary Medical Group (CPCMG) in Oceanside, CA, with some potential for cross-site training.

  • All employees begin at starting rate regardless of experience.

POSITION SUMMARY
The Community HealthCare Navigator works as a team member in a healthcare clinic to provide community resources/linkages to promote health outcomes for clients. Navigators will provide a range of services to include: health education, health navigation, screening and assessments, and support and advocacy to promote overall well-being. Services will include clinic, home and telehealth visits/contacts. Service sites will be Children's Primary Medical Group (CPCMG) in Oceanside, Vista, Escondido, and Euclid Ave (SouthEast).
ESSENTIAL FUNCTIONS
RESPONSIBILITIES

  • Provide health information and coaching to improve health or self-management of health conditions
  • Provide connections to community resources to promote health, address healthcare barriers, receive translation/interpretation services, or address health-related social needs
  • Provide individual or group visits to members that have been referred by a licensed provider for up to 12 visits without prior authorization or development of a care plan. Beyond 12 visits a care plan must be developed in partnership with a licensed provider and authorization for ongoing services must be obtained
  • Provide outreach and resource coordination for preventative services, accessing healthcare, and understanding the health system
  • Participate in community events such as schools, food drives, churches/mosques and other faith-based organizations to provide health and wellness outreach and support
  • Assist assigned communities with identifying and managing obstacles to getting vaccinated and accessing other health and wellness services
  • Provide support to assigned communities while demonstrating appropriate and professional boundaries
  • Attend team trainings including HIPAA, secure online case management system, data sharing and management, reporting, interpretation services and other administrative support

MINIMUM COMPETENCIES
EDUCATION & EXPERIENCE

  • Minimum of a High School Diploma or GED required
  • 3 years experience in Community Outreach, Family Support, Advocacy and/or related services preferred
  • Experience working in community settings
  • Demonstrated familiarity with community linkages and resources in assigned location (north county and/or southeast)
  • Experience with client related data entry
  • Experience working with diverse populations and multiple systems (ie. housing, employment, etc.)

ADDITIONAL EMPLOYMENT REQUIREMENTS

  • Must be 18 years of age or older
  • Must receive Fingerprint Clearance upon hire (paid by Harmonium)
  • Must pass Drug Test upon hire (paid by Harmonium)
  • Must provide recorded proof of the following items upon hire:
  • Annual Flu shot
  • TB test results no older than 1 year
  • TDaP vaccination within the last 10 years
  • 3 Hepatitis B vaccinations or proof of immunity
  • 2 Measles, Mumps, Rubella (MMR) vaccinations or proof of immunity for each
  • 2 Varicella (Chicken Pox) vaccinations or proof of immunity
  • Primary COVID-19 series plus 1 booster
  • Must have reliable transportation valid CA Driver’s license and maintain proof of current automobile insurance
