Schedule: 11am - 7:30pm (Every Other Weekend)

Position Summary

This position provides care to participants aged 55 and older in an adult day center and outpatient clinic setting following the regulations and guidelines established for PACE organizations. The primary responsibilities of the Day Center Aide is to provide personal care, homemaking, and respite support in home environment, and in day center environment as needed, regular collaboration with other members of the interdisciplinary team to ensure patients' care needs are met, assists all participants with activities of daily living (ADL), as needed according to their current level of care, and nursing care plans, and document in the participant's medical record as required by policy. This position may require occasional travel to Porter Hills main campus, other PACE Centers, conferences and seminars.

Required Qualifications

  • High School Diploma/ GED
  • Current Certified Nurse Aide (CNA) certification in the State of Michigan from an accredited program
  • Must have reliable transportation and valid drivers license
  • One year of experience working with the frail elderly, long term care population, or end-of-life care
  • Must have current CPR certification (or willing to obtain)
  • All employment offers are contingent upon successful completion of a pre-employment criminal background check, professional reference checks, pre-employment physical, and TB Screen at time of hire. In addition, proof of COVID-19 vaccination or approved exemption is required.
