JOB DESCRIPTION

I.

POSITION TITLE

DIRECT SUPPORT PROFESSIONAL ONSITE 2

II.

FLSA CLASIFICATION

Non-exempt

SUMMARY This position is a safety-sensitive position requiring the safe operation of a motor vehicle in the provision of public transit services and the effective transportation of all passengers. Performance of these duties must be conducted in compliance with all applicable DOT/FTA, federal, state, local, and system regulations and policies, including U.S. DOT drug and alcohol testing regulations.

QUALIFICATIONS

A. High School diploma or GED, Preferred. Exceptions to educational requirements must comply with state law and regulations.

B. Must have a clean state limited criminal history check and county police check.

C. Will obtain and maintain an Operator for Hire Licenses.

D. Must possess and maintain a good drivers record

E. Must have a negative TB screen

F. Must have no incidents filed in the Certified Nurses Assistant state registry.

G. First Aid and CPR training required after employment begins.

H. Proof of insurance for personnel vehicle

I. Must pass the FTA required drug and alcohol test, no DUI in the last 5 years.

IV.

GENERAL FUNCTIONS

A. Assist individuals with activities that may include any task or tasks of direct benefit to the individual that would generally be performed independently by persons without intellectual/developmental disabilities such as personal care, meals, shopping, errands, scheduling appointments, chores, and leisure activities.

B. Assist individuals with mobility- including but not limited to transfers, ambulation, and use of assistive devices.

C. Escorting the individual to community activities and appointments.

D. Provide transportation for individual.

E. Provide ongoing employment support services that enable an individual to maintain integrated competitive employment in a community setting.

F. Provide to individuals or a group of individuals person-centered, monitoring, training, education, demonstration, or support to assist the individual with acquisition and retention of skills such as leisure activities and community/public events, education activities, hobbies, unpaid work experience, and maintain contact with family and friends.

G. Provide training and education in self-direction designed to help individuals or a group of individuals develop self-advocacy skills, exercise civil rights, acquire skills that enable self-control and responsibility for services and supports received or needed, and acquire skills that enable the individual to become more independent, integrated, or productive in the community.

H. Provide input for reviews of PCIPSs. Be responsible for implementing goals and objectives for clients needing community support and/or participant assistance as outlined in the PCISP.

I. Maintain daily logs of activities with individuals served.

J. Attend in-service trainings as scheduled and complete required online trainings by due dates

K. Other duties as assigned.

V.

ESSENTIAL FUNCTIONS

A. Ability to communicate well enough to train clients in basic daily activity skills

B. Adequate writing skills to complete required forms and reports on daily activities provided.

C. Ability to foster good relationships with persons at community volunteer sites.

D. Ability to follow oral or written instructions.

E. Reliable Vehicle

F. Physical ability to move and stand for prolonged periods and frequently perform physically demanding tasking( such as lifting to transfer/toilet/assist in daily living skills; to provide hand to hand assistance to individuals with mobility issues; to assist individuals in wheelchairs; to bend and squat to secure individuals’ wheelchairs to transport them safely)

G. Ability to transport clients to provide community habilitation and participation.

H. Operate a motor vehicle in compliance with all applicable DOT, federal, state, local, and system regulations and policies and ensures the safe and timely transportation of all passengers.

SUPERVISORY RELATIONSHIPS:

Direct Support Professional Onsite 2 reports to the Day Services On-site Supervisor
