Responsibilities/Duties

• Create and maintain a safe and positive environment.
• Support, guide, and teach consumers everyday skills,including hygiene safety, food preparation, shopping, budgeting, interpersonal skills, utilizing community resources, social and interpersonal development.
• Complete and review all required documentation, including vehicle logs, medication logs, progress notes, sleep logs, menu plans, client inventories, medical/dental forms and etc..
• Collaborate with treatment team in order to properly implement goals listed in Individual Support plans (ISP'S)

Required Experience

• Prior experience working with individuals with challenging behaviors is perferred.
• Paid Training Provided

• Valid Drivers License
• Ability to Pass Mandatory Criminal Background Check

Program Summary

Community Passages is a community-based residential program designed to provide a homelike environment for individuals with Developmental/Intellectual disabilities and Mental Illness. many of whom present significantly challenging behaviors and most of whom previously resided at state centers and hospitals. Residents develop life skills, increase their self-esteem, and are empowered to make choices for themselves.
