Position Summary:
The person in this position will be responsible for the coordination of the overall functioning and program management for the San Joaquin Valley Veterans HVRP (Homeless Veteran Rehousing Program). This position is considered to be a provider of direct services and supervisory responsibilities. Essential Job Functions include those listed below.

Essential Job Functions:
  • Act as a lead case manager and set standards for performance;
  • Responsible for the accuracy, quality, and timely submission of Technical Performance Reports (TPR) and supporting Narratives (TPN)
  • Approves Individual Employment Plans (IEP)
  • Provide training services (orientation, financial management, resume writing, interview skills, etc.);
  • Oversee the distribution of materials, vouchers, reimbursements, and assistance;
  • Coordinate with community providers to insure that a viable network of support services is available to all participants;
  • Conducts community outreach to assure Veterans and Families become aware of opportunities and services to help with employment as a means to maintain permanent housing
  • Develop a Business Council;
  • Integrate EAP services with the activities of other agencies in the community;
  • Work with WestCare finance and clinical services to assure coordination and communication, for the provision of a high level of services Provides overall grant management of the program
  • Provide daily, weekly and monthly reports as requested;
  • Meet often with Case Managers / Job Developers to address any concerns and ensure that best practices are applied
  • Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; and
  • Perform any other duties as assigned.

Certifications/Licenses:
  • Not applicable for this position.

Education:
  • Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or a related field is preferred;
  • GED or High School Diploma is required.

Experience and Competencies:
  • Experience consisting of two (2) years of providing direct outreach services in an intensive setting;
  • Ability to work a flexible schedule (schedule may include weekends and evening hours);
  • Veteran and/or military experience preferred
  • Excellent written and verbal communication skills;
  • Attention to detail and problem-solving skills;
  • Ability to exercise good judgment and discretion;
  • Ability to work well in a team environment;
  • Excellent time management skills and ability to multi-task and prioritize work;
  • Superior organizational skills;
  • Proficient in all Microsoft Office applications;
  • Professional appearance and demeanor;
  • Must be cultural/linguistically sensitive to populations served;
  • Ability to obtain and maintain clearance from LEIE and NSOPW;
  • Ability to obtain and maintain 1st Level Background Checks;
  • Valid driver's license and ability to obtain and maintain WestCares vehicle insurance is required; and
  • CPR Certification, First Aid Certification and an annual Tuberculosis test. As mandated by the State of California those employees who work at Residential, Outpatient, Sober Living and provide Mental Health Services or Transitional Living Facilities must be fully vaccinated by September 30, 2021. Employees that have requested a Medical or
  • Religious Exemption, which has been approved by Human Resources will be required to test for the virus weekly.
  • During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you

Working Conditions:
  • Work is primarily performed in an office or treatment facility setting; and
  • Some outdoor activities may be required.
