Our extended primary care team includes district nurses, physiotherapists, occupational therapists and support workers. We support patients who are house bound, at risk of hospital admission or nearing the end of their lives, working in partnership with them and also with their families and their carers.We are looking for a Health Support Worker to work alongside our district nurses, providing high quality and compassionate care for our patients.
We are a friendly and welcoming team, based in the south of Tower Hamlets (Poplar / Isle of Dogs area).

To communicate effectively and in a professional manner at all times with key stakeholders, members of CHT, patients and their carers or advocates to promote seamless care. Adhere to Caldicott guidance and maintain standards of confidentiality as outlined by Trust Policy and information governance guidance.
To have good communication skills, utilising tact, empathy, compassion and reassurance including communication with people who are dying, those who experience mental health issues, cognitive or sensory impairment, learning difficulties and people whose first language is not English and require interpreters. Be able to gain informed consent prior to treatment of patients and be able to work with patients or carers to overcome barriers to care.
To encourage patients to maintain optimum independence through participating in all decision making related to their care. This will involve talking to them about their needs and preferences in relation to activities such as eating and drinking, meals, washing and dressing, sleeping, and taking part in individual, and group therapy activities.
To resolve any queries or problems that are raised by patients or their carers as they arise wherever possible or to ensure their key worker or a senior member of the team is informed to resolve the concerns.
Demonstrate customer care skills when listening to concerns, dealing courteously with visitors, staff or patients at all times, including during telephone conversations.
