About the Organization and Position

Well known Non-Profit Health Agency is hiring for a Healthcare Case Manager / Outreach Specialist.

This wonderful non-profit serves people age 60 and older, adults living with disabilities, and their families and caregivers in Lynn, Lynnfield, Nahant, Saugus, and Swampscott, Massachusetts for more than 45 years. They provide a continuum of community health and social services to help people maintain their independence, safely and with dignity. Providing a broad range of services including home care services; nutrition programs; transportation assistance; housing supports; clinical and protective services etc. The agency recently received a second prestigious 3-year accreditation for excellence in case management and care coordination from the National Committee for Quality Assurance (NCQA).

The Outreach Specialist will initiate outreach calls to referred individuals and Provide information about the option to enrollee, including a description program and the enrollment process to manage program enrollment. The Outreach Specialist will assist with the management of Assignment files.

Responsibilities

  • Provides Outreach support for the program.
  • Initiates initial outreach call to potential enrollees.
  • Provides information about the option to enrollee, including a description of the program and the enrollment process.
  • Obtain Participation Forms from enrollees who agree to participate in the program to confirm consent
  • Schedules and performs home visits and coordinates outreach follow-up letters
  • Maintains Outreach documentation in eHana.
  • Tracks outreach calls and unscheduled home visits
  • Enters visit into CPC’s calendar.
  • Prepares package of information for home visits according to Enrollees site location.

Experience

  • Associates Degree minimum, Bachelor’s Degree preferred from
    accredited institution of higher learning.
  • Minimum of 5 years’ experience working with individuals of all ages and living with various disabilities
  • using a consumer directed approach.
  • Ability to attend and complete required trainings.
  • Must be organized, able to multitask, work within a team in a fast-paced environment and be self-directed.
  • Exceptional interpersonal and assessment skills are required in order to make well
    informed decisions.
  • Proficiency with Microsoft Word, Excel, Outlook,
    and PowerPoint required.
  • Strong writing and communication skills essential. Must have valid driver’s license and reliable transportation.
