Home Care RN
Overview: RN will complete in-home nursing assessments of geriatric and high-risk patients within (1) assigned borough of choice to aid in preventing hospital readmission.
Responsibilities:
- Perform physical exams and health histories before making critical decisions.
- Provide health promotion, counseling and education on after care.
- Administer medications, provide wound, trach care and other personalized interventions as needed.
- Coordinate additional care, in collaboration with a wide array of health care professionals.
Requirements:
- NYS Registered Nurse
- 2 years of Home Care or palliative Care Experience
- AHA BLS
- Proof of Full Covid-19 Vaccination
- Ability to complete a minimum of 5 visits per day
Job Types: Full-time, Contract
Salary: $2,800.00 per week
