We are currently looking for a motivated part-time or PRN Home Health RN Case Manager for our patients in the West Valley. Our Home Health RNs plan, organize and direct care of patients from their home setting to achieve the most positive health outcomes and decrease the likelihood of rehospitalization.

This RN has the flexibility to create their own schedule based on patient and community needs while meeting the requirements as designated in the physician order. Utilizing their depth of experience and knowledge, the home health nurse is responsible for:

• Completing an initial home health assessment of patient and family to determine home care needs
• Conducting in home visits as assigned, assessing the patient’s condition, progressing towards goals, providing patient care, teaching and other skilled interventions as ordered by a physician
• Evaluating patient care needs
• Developing care plans based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions
• Administering medications and treatments as prescribed by the physician
• Counseling and teaching the patient and their family in meeting needs
• Completing supervisory visits of team members on a timely basis as required by CMS regulations

Requirements:

• Valid AZ RN license (or nurse compact license) (required)
• Minimum of two years of Home Health nursing experience (required)
• BSN (preferred)
• Experience with a home health electronic medical record system (required); knowledge of Axxess electronic medical record system (preferred)
• Valid AZ Driver’s License and automobile insurance (required)
• Reliable transportation (required)
• Pass background check (required)
