Job description
Assists the Director in the administration of rehab services thru supervision of therapy staff in the home heath setting. Is responsible for developing daily work assignments, flexible staffing, performance improvement, orientation, staff development, and planning and budgeting. As a Therapist, evaluates patients and develops a treatment plan for patients suffering from injuries, neurological conditions, or muscle, nerve, joint and bone diseases, to restore strength, movement, function, relieve pain, and prevent disability.
Responsibilities:- Responsible for the supervision of all therapy disciplines in Home Health/Hospice
- Coordinates multi-discipline meetings, promotes and develops programs to customers and the community
- Assist with the development of new programs designed to meet the needs of our patients
- Reviews monthly financial reports and quarterly productivity data and takes action as needed
- Consistently focuses on efficient use of resources
- Plans and records agendas/minutes for all rehab meetings
- Assists in development of policies and procedures, performance appraisals and time maintenance
- Plans meetings, in-services, orientation, and educational competencies for staff
- Interviews and assists in the selection of new partners
- Plans and coordinates the orientation and initial competency assessment of new therapist
- Investigates and assists in formulating response on patient concerns
- Reviews and analyzes McKesson reports to improve accuracy and completion of documentation
- Reviews therapist time reports and mileage for accuracy and approves time cards bi-weekly
- Review reports to determine therapy re-assessments needs per regulations
- Assist the Director with accuracy of Oasis scoring at SOC/Transfers/Re-certifications/ROC/Discharges as necessary
- Assist the Director with reviews and approvals of all therapy SOC’s for proper scoring and coding
- Acts as a mentor and leader in regard to McKesson documentation and charting
- Organizes and directs PI groups to assure progress and process changes as needed
- Assures compliance with regulatory and accreditation requirements
- Assists as needed with reviews of Physician's referrals, patient's condition and medical records to determine therapy treatment required
- Monitors time and travel along with management/approval of payroll for all therapy staff
- Obtains history and subjective components of the evaluation
- Measures patient's range-of-joint motion, grip/pinch strength, sensation, coordination, muscle strength to assist with the evaluation
- Completes evaluations and treatment addressing, functional mobility, ADL activities, functional, strength, and cognitive deficits
- Completes an assessment of patient's current status, develops goals and a treatment plan with recommendations
- Plans and implements therapeutic programs and activities designed to restore, reinforce, and enhance task performances, diminish or correct pathology, and to promote and maintain health and self-sufficiency
- Instructs, motivates, and assists patients to learn and improve functional activities and daily living activities
- Recommends, designs and adapts equipment for the working and living environment
- Effectively and appropriately utilizes modalities as part of a treatment plan
- Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit
- Performs manual therapy, soft tissue mobilization, joint mobilization
- Utilizes evidence-based knowledge to provide most appropriate therapeutic intervention
- Records evaluation, treatment, response, and progress in patient’s electronic medical record
- Confers with Physician and other practitioners to obtain additional patient information, suggest revisions in treatment program, and integrate therapy treatment with other aspects of patient's health care
Minimum of five years of experience as a Therapist
Minimum one year of Home Health experience
Certification/Licensure:
Oasis or other approved therapy certification preferred
Approved CPR or BLS certification required within 90 days of hire

While performing the duties of this Job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to stand; walk; sit and stoop, kneel, crouch, or crawl. The employee is occasionally required to climb or balance and taste or smell. The employee must frequently lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision and ability to adjust focus.
