Job Summary:

The Medical Social Work (MSW) professional provides skilled services to patients in their homes to promote the patient’s plan of care. These services are performed in accordance with the physicians’ orders and in collaboration with other Haven Hospice therapists, nurses, and clinicians.

Duties and Responsibilities:

• Develop and implement an individualized plan of care related to MSW in consultation with the patient's physician and Hospice physician
• Re-evaluate and revise the patient's plan of care as necessary
• Counsel the patient and family in meeting MSW and related needs
• Assist team members, including the physician, in understanding the significant social and emotional factors related to health problems

Qualifications, Knowledge, Skills, and Abilities:

• Masters Degree in Social Work
• Minimum of one year experience as an MSW within the last three years
• Flexibility in work schedule to accommodate patient needs, including new admissions
• Strong clinical judgement, assessment, communication, and leadership skills
