Immediate Access to Care Coordinator - Springfield
Job description
Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. Essential Duties:
- Conducts SUPR Programs intakes, including; MAR outpatient; intensive outpatient; residential; halfway house; Quincy satellite, DUI; DCFS; Medicaid/MCO’s, SOR, Hub and Spoke and any other Special Projects
- Answers daily inquiries of patients and public in person or via telephone regarding admission procedures, agency regulations and services; when necessary, refers to appropriate person or department elsewhere in the agency; provides directions for finding other areas within the agency
- Screens treatment inquiries and triages for assessment/admission based on need per ASAM Patient Placement Criteria
- Makes appropriate referrals to other SUD treatment programs when appropriate treatment services are not available at agency
- Makes appropriate referrals to ancillary agencies when special needs are required
- Handles patient referrals of an urgent nature, both internal and external
- Coordinates scheduling for Admission Assessments for patients seeking MAT, Residential and Outpatient services with Admission Specialists and/or Admissions Manager
- Coordinates with patients and/or representative while in waiting for Admissions Assessment and continues coordination throughout the Admission process
- Acts a liaison with community referral sources and FGC Outreach staff for availability of admissions and scheduling of admissions
- Maintains patient confidentiality at all times in accordance with 42 CFR and HIPAA regulations
- Other duties as assigned
Schedule: Monday - Friday (8:00a - 4:30p)

Experience and Skills:
Education and/or Experience: High School Diploma required. Associate degree or higher preferred.
Certification, Licensure, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) or Certified Recovery Support Specialist (CRSS) required within allotted time of hire.
