What You Should Know About the RN Care Manager Role:
- The Float LPN/RN is a DRIVING position and will require travel throughout the Montgomery/Butler/Warren counties
- This is a PRN position, day shift hours 8:00A-5:00P

The LPN/RN Float Essential Duties:
The LPN/RN Float will assist Care Partners, Coming Home, as needed. Float team staff will collaborate with various IDT members both internal and external of the organization. Clinical services will be scheduled in a timely fashion according to the patient’s plan of care, observing patient’s continuity of care to achieve the desired outcome of comfortable and peaceful death for patients. When in the field, staff will provide excellent clinical care to all Ohio’s Hospice patients that meet the needs of our patients/families, Ohio’s Hospice standards, Medicare Conditions of Participation and achieve the desired outcome of comfortable and peaceful death for our patients.
RN/LPN Qualifications:
RN's - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred. Active Registered Nurse license in the State of Ohio without any board actions. Two years acute care nursing experience preferred.
LPN's - Registered LPN with the State of Ohio without any board actions. One-year nursing experience preferred.
- CPR Certified
- Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
- Computer skills sufficient to properly document services and care.
- Ability to drive during daytime, nighttime or inclement weather.
- Valid Driver's License with Safe Driving Record
- State Minimum Automobile Insurance Coverage
