Brighton House Nursing and Rehabilitation is seeking a Licensed Social Worker to join and support our fantastic team caring for our short-term patients and long-term residents. Accessible by public transportation, this this facility is located in the heart of Brighton, nestled in a beautiful neighborhood between Commonwealth Avenue and Beacon Street. Requirements for Licensed Social Worker:
- Must possess a minimum of a B.A in Social Work, LSW or equivalent degree with a current social work license.
- Masters in Social Work (MSW), and LCSW or LICSW higher preferred.
- 1+ years skilled nursing facility experience preferred
- Job Duties of License Social Worker:
- Complies social history data
- Participates in assessments, development and evolution of residents care plan.
- Responsible for planning and leadership interdisciplinary care planning weekly meetings.
- Documents timely progress notes according to state and federal regulations.
- Coordinates the discharge planning for residents.
- Able to interpret and implement established social service policies, procedures and regulations.
- Coordinates services of community agencies for continued delivery of care upon discharge.
