Occupational Summary
This position is part of the Office of Aging and Adult Services (OAAS) Program Operations section and is domiciled at the OAAS Regional Office in Monroe (Region 8). Local travel and field work is required in order to perform field visits to Nursing Facilities, Provider Agencies, Support Coordination Agencies (SCAs), and participants’ homes to ensure OAAS Long Term Care programs serving the elderly and persons with adult-onset disabilities are delivered efficiently, effectively, and within program rules.
The ideal candidate will have excellent written and verbal communication skills, possess analytical thinking and problem-solving skills, demonstrate initiative, and be well organized. The position also requires the ability to: engage and develop a rapport with others, apply knowledge and think critically, promote change and adaptability, and organize and prioritize workload. The candidate must be/become certified in the use of the interRAI Home Care (iHC) assessment tool and in person-centered care planning principles.
The following attributes are preferable (but not required): experience using standardized functional/clinical assessment tools, knowledge of Medicaid Long Term Care Programs’ rules and regulations {including Nursing Facilities, Community Choices Waiver (CCW), Adult Day Health Care (ADHC) Waiver, Long-Term-Personal Care Services (LT-PCS) and the Program for the All-Inclusive Care for the Elderly (PACE)}, proficiency in MS Office Suite (Word, Excel, Power Point, etc.), Microsoft SharePoint, and familiarity with Medicaid’s LaMEDS, SIMS system, and LDH OAAS Participant Tracking System (OPTS).
Primary duties include:
- Conducting annual SCA monitoring to ensure quality services are delivered effectively to participants.
- Preparing and submitting cases to the OAAS Service Review Panel (SRP) for review/case guidance.
- Issuing accurate denial/discharge notices to individuals/participants timely and per policy guidelines.
- Completing Summary of Evidence (SOE) packets for appeal hearings and participating in the hearings representing the Department.
- Monitoring and completing Critical Incident Reports (CIRs) until the incident is resolved and the participant summary and final report can be completed, compliant with CMS timelines.
- Conducting home visits with individuals/participants, including expedited waiver offer requests, resolution meetings, plan of care meetings, team meetings, monitoring visits, etc.
- Providing technical assistance to Direct Service Providers (DSPs) and SCAs as necessary.
- Reviewing Nursing Facility Level of Care Reviews with Nursing Facility residents for Level of Care determinations.
- Attending and participating in quarterly SC and DSP meetings, as needed.
- Responding and being available during statewide disaster/weather events (potential or otherwise) to report to Emergency Assignment.
- Participate in monthly SCA meetings to update Aging Reports and follow up on My Choice Louisiana and Money Follows the Person.
Additional duties may include:
- Participating in meetings, possibly after hours, to provide support and be available for questions that may arise.
- Reporting to assigned LDH emergency response location when activated for emergency response duty assignments.
- Conducting iHC assessments in the field at individuals’ homes.
- Other duties as assigned.
As part of a Career Progression Group, vacancies may be filled from this recruitment as a Medical Certification Specialist 1-2 depending on the level of experience of the selected applicant(s). The maximum salary for the Medical Certification Specialist 2 is $106,392.
