Alameda Healthcare and Wellness Center is looking for a compassionate, competent and committed LVN Case Manager/Social Services to join our team.

Purpose: To work passionately and holistically to ensure that the care and discharge plans meet the physical, social, and emotional needs of our patients.

Duties and Responsibilities:

  • Coordinate patient care and services.
  • Coordinate, monitor, and evaluate services according to the needs of patient.
  • Provide advice and recommendations to the family of a patient on ways to support a loved one through the recovery process.
  • Serve as a liaison between a patient and his/her healthcare provider to ensure clients receive quality health care service.
  • Ensure that healthcare staff fulfill their obligations and responsibilities by making sure patients receive both medical and psychological treatment.
  • Ensure proper documentation of client case management plans and treatment progress.
  • Educate patients on their health status as well as proffer recommendations on steps necessary for quick recovery.
  • Provide advocacy services to assist impaired patients in accessing government healthcare aid.
  • Work to promote a safe transition from the facility to the next provider or care setting in accordance with all applicable laws, regulations, and Life Care standards.
  • Make independent decisions when circumstances warrant such action
  • Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
  • Implement and interpret the programs, goals, objectives, policies, and procedures of the department
  • Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation

Requirements and Qualifications:

  • Currently licensed/registered in applicable State.
  • Must currently hold and maintain an active LPN/LVN license in good standing throughout employment.
  • May be required to obtain certifications from the American Case Management Association (ACMA) or the Commission for Case Management Certification
  • One (1) year of experience in post acute care setting preferred
  • Prior case management, utilization review, and discharge planning experience preferred
  • Generate written communication that is clear, concise, and well organized
  • Excellent organizational skills and be efficient in prioritizing and managing time and assignments
  • Maintain professional working relationships with all associates, vendors, etc.
  • Maintain confidentiality of all proprietary and/or confidential information
  • Understand and follow company policies including harassment and compliance procedures
  • Displays integrity and professionalism by adhering to Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training

Physical Capacities: (With or Without the Aid of Mechanical Devices)

  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
  • Must be able to move intermittently throughout the workday.
  • Continuous bending, squatting, twisting and reaching.
  • Must meet the general health requirements set forth by the policies of this facility which includes an annual TB screening and physical examination.
  • Must be able to stand for and walk for extended periods of time.
  • Must be able to lift and carry up to 25 pounds.
  • Requires infrequent lifting of weight up to 100+ pounds with assistance.
  • Frequent pushing and pulling of carts and transporting residents.
  • Must be able to cope with the mental and emotional stress of the position.

Environmental conditions: Inside work, normal temperatures, some noise, occasional fumes/odors, chemical exposure and potential exposure to bloodborne pathogens.

Pay: $40.00 - $45.00 per hour
