OPS Senior Community Health Nurse


Your Specific Responsibilities:


This position is a Registered Nurse providing health care to the homeless at the Bayside Clinic as part of for the Health Care for the Homeless Program. Duties of the position include:


  • Assess and evaluate clients within the scope of State and Federal guidelines, Florida Department of Health in Pinellas County policies, and the Nurse Practice Act.
  • Utilize nursing skills of interview, observation, and assessment to determine the medical needs of the client.
  • Use the nursing process to plan for ongoing assessment of risk factors and service as prioritized by the client being seen.
  • Complete medical history intake and updates.
  • Provide direct services to clients including wound care, immunizations, administration of ordered medications, vital signs and other testing or services which fall within the nursing practice.
  • Perform venipuncture, urine collection and finger sticks. Prepare specimens for transport to outside labs.
  • Provide case management to established or referred clients in need of nursing intervention.
  • Provide and document health education, referrals and follow up.
  • Ensure a safe and healthy environment by adhering to OSHA guidelines for the prevention of exposure to blood-borne pathogens, utilization of universal precautions. Prepare for and handle emergencies and treat according to standing orders as needed.
  • Counsel clients in basic health principals and risk behaviors, including but not limited to communicable disease control, tobacco use, nutrition, and safety.
  • Consult with provider regarding client needs, concerns or problems and assists providers as needed.
  • Enter health information and other pertinent data into EHR.
  • Maintain confidentiality of all records, client interactions, and services according to HIPAA privacy policies.
  • Prepare clinical area for services.
  • Participate in quality improvement activities, including chart review, in service programs, and staff meetings.
  • Perform other related duties as assigned including emergency duty.

  • Ability to work with persons of a different race, language, culture, or socioeconomic status
  • Ability to adapt to necessary changes in duties, duty station, or work hours for the benefit of the agency
  • Skill in one-on-one interviewing

Qualifications:

Minimum-


  • Clear and valid Florida Nursing License
  • Florida Driver’s License and access to reliable personal transportation

Preferred-


  • Five or more years of experience working as an RN in an internal medicine or family practice clinic setting
  • Five or more years of experience working as an RN in an outreach setting
  • Experience providing wound care
  • Experience working at a Federally Qualified Health Center
  • Experience providing clinical care to patients experiencing homelessness
  • Experience with Telehealth
