POSITION INFORMATIONPosition summary:Responsible for providing direct care to homebound patients requiring skilled care. Works under the supervision of a Physical Therapist in managing a caseload of patients by providing therapy treatments under the established Plan of Care.

MINIMUM QUALIFICATIONSEducation:Graduate of an accredited Physical Therapy program. Associate’s degree required.
Required length and type of experience:Prefer minimum one (1) year experience in a home therapy role.
Required licensure, certification or registry:Current Ohio PTA license. Current CPR certification.Special Qualifications:Demonstrates effective problem solving/decision making abilities. Excellent communication skills. Demonstrates the ability to plan and prioritize with excellent time management skills. Performs with initiative and innovation. Knowledge of practice requirements for Physical Therapist assistant.

Schedule: FULL-TIME, Days, 8:00 am - 4:30 pm
