Position Summary

Directs the assessment, treatment planning and implementation of therapy services to patients of all ages, utilizing current knowledge, theory, evidence and sound clinical judgment. Works closely with the patient, family and other members of an interdisciplinary team to provide efficient and effective individualized patient care. Serve as educators and mentors for therapy students, peers and other medical professionals.

Key Responsibilities:

• Conducts an appropriate evaluation of the patient and documents accurate information within the medical record
• Develops and implements age appropriate functional goals and treatment plan based on referral, the patient’s history, examination findings, and patient’s input
• Develops appropriate treatment interventions, considering indications and contraindications, based on patient’s current level of functioning
• Provides treatment at a level consistent with current therapy practice
• Delegates and provides appropriate supervision of established plan of care to support personnel.
• Completes accurate and timely documentation of the patient’s progress and the treatment performed that meet professional standards.
• Communicates/collaborates with interdisciplinary team members regarding patient progress, response to treatment, goals and program changes.
• Casual/call positions have specific requirements regarding coverage expectations.

Selection Criteria:

• Licensed by the Minnesota State Board of Physical Therapy to practice in Minnesota
• Prefer 2 years of acute/subacute/homecare expereince
• Current Basic Life Support (BLS) Certification through the American Heart Association

Schedule:

• Varying Days/Shifts
• Shift start and end times vary between 8:00am and 5:00pm
• Number of Hours Required: 32 hours per calendar year minimum
• One weekend per quarter required
• No holidays required
