Job description
POSITION OBJECTIVE: Identify and assess client and family needs related to the intake, placement, and permanency of children placed in care or under supervision by DCF/NWFHN due to abuse or neglect by caretakers. Evaluate, coordinate, and ensure necessary services and data integrity.
ESSENTIAL FUNCTIONS MAY INCLUDE:
· Staff DCF referrals for services with Protective Investigator, Child Welfare Legal Services,
Case Manager, and Case Manager Supervisor to determine case acceptance. Assess appropriateness of referrals and risk level from DCF.
· Ensure eligibility determination, demographics, placement, legal status, etc. is accurate in FSFN. Review cases and transfer as appropriate.
· Provide on-call support as required or scheduled
· Monitor case plan progress and evaluate reports related to permanency and best practice.
· Develop and maintain case and program documentation according to contract and agency standards. Keep all eligibility/placement files up to date and accurate - ready for on-the-spot monitoring.
· Oversee diligent searches as required by dependency laws
· Assist with the coordination of services with other professionals and paraprofessionals
· Conduct community outreach as needed
· Provide specialized services. Assist family/caregivers in accessing subsidy, medical coverage and other needed services for children.
· Coordinate case staffing’s with Case Manager Supervisor, Case Manager, providers, DCF and Child Welfare Legal Services as needed. Communicate with agency partners as necessary.
· Facilitate placement by maintaining good relations with foster home/ facility providers.
· Complete all necessary paperwork (placement information form, chronological entries, waivers, funding requests, etc.) related to the placement of children. Show diligence in locating appropriate placements for children in and entering licensed care.
· Exhibit expertise in Placement, as well as Revenue Maximization· Participate in the quality improvement process. This list of essential functions is not intended to be exhaustive.

QUALIFICATIONS:
REQUIRED:
- Bachelor’s degree from an accredited university
PREFERRED:
- Previous experience in a social services organization
- Child Protection Professional Certification.

- Ability to observe and report accurately on the functioning of individuals and families
- Ability to handle confidential information appropriately

- Ability to make home visits and to travel
- Ability to set appropriate limits and boundaries with clients
- Knowledge of community resources
- Ability to work flexible hours as required
- Knowledge of professional ethics relating to child protection and counseling
- Skills in employing family-centered practice
- Ability to use and proficiency in computer software applications
- Ability to act decisively when necessary to protect clients
