Description:

Practice Coach-Office Specialist is a highly visible position within provider offices affiliated with the Trinity Health Alliance of Michigan. The incumbent has the responsibility of training, educating, and working with Practice staff to re-engineer processes and procedures relating to improving health outcomes, patients’ experience of care and practice efficiencies in support of community and payor contracts held by Trinity Health Alliance of Michigan. The Practice Coach-Office Specialist is dedicated to working with physicians and physician office staff to improve performance on key outcomes which drive consistent achievement of the Quadruple Aim goals.

What the Practice Coach will do:

  • APM Initiatives/Clinical Integration/Credentialing Activities
  • Support AHN Performance Initiatives by participating in process improvements to support total cost of care/quality outcomes, including data collection and medical office chart audits.
  • Cascade relevant information to the physicians and office staff about clinical initiatives, quality programs, and other medical studies, completing documents for quality studies with physicians and their office staff.
  • Hold regular coaching sessions, typically face to face, with identified Network physician members to help them meet various internal total cost of care goals, share performance data and results, and jointly develop improvement action plans.
  • Coaching sessions (at times in conjunction with Care Manager and/or Pharmacist) may include the following topics: reviewing office performance results, risk adjustment, tactics for improving results (clinical or efficiency), tactics for avoiding ambulatory sensitive admissions, excelling performance in the clinical integration program measures as well as total cost of care, advanced payment model contracts, or other AHN contracted measures, and related topics.

What the Practice Coach will need:

  • Must possess a comprehensive knowledge of Physician Relations, as normally obtained through a Bachelor’s degree in Business, Healthcare Administration, Marketing or related field from an accredited college or university.
  • Minimum of five years of progressive experience in healthcare working with physicians and healthcare leaders, including senior management.
  • Ability to develop proactive partnerships with physicians, practice managers and staff, peers, senior leaders and other stakeholders.
  • Proficiency with or ability to learn clinical information systems supporting PRM (Physician Relationship Management tools). Advanced knowledge of Microsoft Office Products required.
