Job Summary:
The Remote RN promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members and providing physical and psychological support to patients, friends, and families. Adheres to and practices the SJH Corporate Compliance Program and participates in Performance Improvement activities. The primary duties of this job are scheduling colonoscopies, patient care management and maintaining Referrals for General Surgery and Gastroenterology.

Essential Functions:
1. Patient Care
  • Obtains patient consent for care and ensures patient confidentiality. Obtains and documents health history, information on chief complaint and health risk factors. Assesses disease condition using observational, interviewing, interpretation, and critical thinking skills in a developmentally appropriate manner.
  • Provides patient care based on practice guidelines, standards of care and federal/state laws and regulations.
  • Executes prescribed treatments and medical interventions, administers prescribed medications and monitors and documents treatment progress and patient response.
  • Delegates as allowed under the Nurse Practice Act.
  • Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices.
  • Talks directly to patients on the telephone and then directs them to emergency rooms (ERs), urgent care centers, or to see their physician during office hours.
  • Performs short evaluation of the patient situation to estimate severity of illness and/or injury including learning about chief complaint and, as possible, and mental status information.
  • Performs other duties as assigned
2.Care Management
  • Collaborates with other clinics, community resources, providers and staff as needed in a timely and efficient manner. Ensures continuity of care.
  • Educates patients and families about health status, health maintenance, and management of acute and chronic conditions.
  • Participates in multidisciplinary teams to improve patient care processes and outcomes. Tracks quality assurance data and monitors for acute and chronic care management.
  • Ensures accurate notes of all consultations and treatments are recorded in the patients' record via the clinical computer system.
  • Arranges appointments for patients who do not need to go to ER but need to see a physician. Consults with physician as needed.
  • Completes and tracks prior authorization status for patient medications, procedures, high-cost drugs, and completes appeals as necessary to reduce hospital write-off costs.
  • Manages the referrals in referral management and self-referring patients. Call patients in a timely manner and schedules procedures for patients. Updates the status of the referral and notes any pertinent information in the patient chart.
  • Sends surgical bookings and coordinates with OPS, providers and General Surgery RN's.
  • Once the patient is seen by the provider a note of that visit/ op note is sent back to the referring physician.
  • Performs other duties as assigned
3. Clinic Support
  • Promotes a team-based model by fulfilling virtual nursing duties as assigned.
  • Be able to work between three Cerner message inboxes to assist with triage, refill medications per protocol, patient result notification, etc.
  • Assist with Medicare Annual Wellness visits
Job Requirements: Minimum Education:
Required: ASN
Preferred: BSN

Minimum Work Experience:
Required: Must be able to perform specific clinical tasks while providing excellent clinical care. Must possess sound nursing knowledge and be able to provide/assist in patient education. Must have sound knowledge of clinic procedures. Ability to read/interpret physician notes and orders, analyze options, and provide counsel to patients. Triage patient need/concern over the phone.
Preferred: Two years of experience as an RN in an ambulatory setting

Required Licenses: Current RN License with the authority to practice in the State of Wyoming and BLS required at the time of hire.

Functional Demands:
Working Conditions: Normal office hours, 3 days per week in a virtual office setting. Frequent interruptions and stressful situations may present throughout the day. Virtual contact with patients, coworkers, insurance companies, vendors, and community members under a variety of circumstances. Occasional overtime may be required and/or hours may be shortened as business needs. Must be willing to attend and participate in regularly scheduled clinic staff meetings virtually.

Physical Requirements: Requires sitting and standing associated with a normal office environment. Manual dexterity is needed for calculator and computer use. Intermittent walking, standing, lifting, bending, stretching. Must be able to lift 20 pounds. Must possess sight/hearing senses to meet the requirements of this position.
