- We are looking for a compassionate, caring individual with a strong background in geriatrics and wound care. We are looking for an experienced professional who is energetic, dependable and able to work in a fast-paced rehabilitation center. The successful candidate will be responsible for performing round-the-clock medical screenings, administering medications and providing comfort to patients. Come join us! . Responsibilities:
- Work as part of a team.
- Demonstrate proper infection control measures.
- Be caring and compassionate toward the sick or dying.
- Coordinate activities with other members of the healthcare team.
- Administer medication to patients in accordance with the doctor's orders.Monitor, record, and communicate changes in patient's condition and respond as appropriate to the needs of the patient and family.
- Coordinates and Assesses MDS and Raps Programming
- Monitors Prom Asessments
- Side-Rail, Fall, Oral, Dehydration,restorative Programming-dsg, amb, transfer, bed mobility, eating, AROM, Prom, splint (writing programs.
- Monitors C.N.A. trackinjg forms
- Monitors ADL tracking forms
- Inservicing on Restoring ADL programs ADL Care programs
- Provide inservices for staff regarding Rehab/Restorative programs
- Assessments
- Care plans
- Falls
- Restraints

Job Type: Full-time
Pay: $38.00 - $40.00 per hour
