We are seeking Skilled Nurses (RN & LVN) for Home Health visits to patients' homes.

Looking for staff to visit patients in Santa Clarita and surrounding areas.

Job Description:

• Provide one-on-one skilled nursing services within patients’ home based on POC frequency and instructions.
• Monitor and document reactions and patient progress using observation, assessment, and evaluation skills.
• Educate patients and family members according to disease process, medications, treatment plans, and best practices
• Report to physician and Case Manager of adverse findings.
• Follow infection control policy in and out of the office.
• Document visits in EMR system (Wellsky) in a timely manner.

Job Requirements:

• Fluent in English (Bilingual is a plus)
• Current California License for Vocational Nurse or Registered Nurse.
• Current driver’s license with auto insurance and transportation.
• Current CPR Certification.
