To qualify, candidates must have one year of service as a Licensed Master Social Worker 2, and have possession of a license and current registration as a licensed master social worker in New York State.
NOTE: If three or fewer qualified and interested promotional candidates respond to this notice, the agency may, at is discretion, nominate a qualified employee for non-competitive promotion under the provisions of Section 52.7 of the Civil Service Law. To qualify, you must be a current OPWDD employee and have one year of service as a Licensed Master Social Worker 2.

Duties Description
PRIMARY DUTIES AND RESPONSIBILITIES TO INCLUDE, BUT NOT LIMITED TO:
  • Support both the clinical and administrative issues involved in the operation of the Placement Coordination Unit (PCU).
  • Coordinate and facilitate regular staff meetings of social work staff who are involve residential placements.
  • Assist the PCU with the day-to-day activities of placement coordination, through regular communications and team meetings with the 6 regional placement coordinating liaisons.
  • Provide expert clinical consultation and input related to transitional care planning for service recipients identified as eligible for a variety of residential movement opportunities
  • Serve as back up in the absence of the Director of the PCU Unit, ensuring continuity of operations of the PCU.
  • Assist with time sensitive State Operations residential placement requests to ensure the 6 regional liaisons are quickly engaged in coordinating the necessary residential opportunities within specified timelines.
  • Coordinate with other Divisions, state partners, and community resources, the transition of individuals to and from the State Operated residential programs and services.
  • Assist the PCU in development and management of a statewide approach and coordination process to liaise with the Division of Service Access, Program Implementation and Stakeholder Support (SAPSS) staff to facilitate flow of individuals with lower acuity of need to more appropriate levels of care, and/or to voluntary providers.
  • Assist with trouble shooting individual needs and coordinate urgent and routine placement activities as needed.
  • Assist with development and maintenance of a detailed tracker showing entrants to service and attrition from State Operated residential services.
  • Assist State Operated services in addressing transitional barriers of high need individuals passing through our residential programs and reintegrating into new communities.
  • Provide expert assistance and guidance to social work staff and other staff involved in placement coordination on the multidisciplinary and clinical teams, who conduct assessments and reviews of recipients of OPWDD services in residential settings.
  • Utilize available data systems including TABS, EHR, SharePoint and related to generate routine, and ad hoc reports to the PCUs needs.
