Responsibilities:

Work with a team of transplant professionals to provide care in a multidisciplinary setting. Perform psychosocial evaluations on pre-transplant or pre-VAD patients to assess their feasibility to transplant. Provide ongoing assessment of the patient and families ability to handle this complex procedure and ability to provide care. Facilitate support groups and community education. Work with community organization to provide for the patient s psychosocial needs.

Qualifications:

• Graduation from an accredited graduate school of social work with an MSW (Master s in social work) degree.
• State licensure is required by the NJ State board of social work examiners.
• Licensed social worker (LSW) or licensed clinical social worker (LCSW) depending upon years of social work experience and clinical course work (LSW may render clinical social work services under the direct supervision of an LCSW).
• Three or more years of experience in a hospital setting is preferred.
• Successful completion of all Orientation Programs.
