Job Summary and Qualifications
As a hospital social worker you will assist our patients in dealing with the effects of hospitalization. Patients and families are often under a lot of emotional pressure and financial stress due to illness or injury. Additionally, there are needs that need to be addressed after discharge such as home care or rehab services. You will help our patients and their families navigate the process of illness and injury, from hospitalization to recovery.

Responsibilities:
- Social Work staff identify at an early stage of hospitalization all patients who are likely to suffer adverse health consequences upon discharge if there is not an adequate discharge plan
- The Social Worker will arrange for any post-acute care needs as they are identified. Other Social Work services may include providing crisis intervention, referrals to community agencies, counseling patients and families and leading support groups
- Work may be stressful at times and it can involve dealing with angry or upset people
- Social Workers are available to provide services to all patients who are in the Emergency Department or are in the hospital as inpatient status, observation status and outpatient in a bed status
- Social Workers are in house seven days a week. Social Work is on call after business hours and on holidays
- Adheres to hospital’s behavioral competencies and department and organizational goals
- Performs discharge planning assessments for high risk patients or any patient identified as needing a discharge planning assessment
- Refers patients to appropriate community resources
- Provides crisis intervention and emotional support to patients and families
- Coordinates patient/family physician conferences
- Participates in interdisciplinary team discussion to develop a patient specific discharge plan that meets the patient’s individual needs
- Develops and maintains working relationships with community agencies
- Documents in patient record per corporate and hospital policy
- Participates in department and hospital performance improvement activities
- Reports any cases of suspected or confirmed neglect or abuse
- Counseling and Support group participation as appropriate
Qualifications You Will Need:
Graduate of an accredited school with Masters Degree in Social Work- Must be licensed in the state of Kansas
- Must have knowledge of community resources relating to health care needs
- Prefer a minimum of one years’ experience in discharge planning in an acute care hospital or in a post-acute care facility (i.e. skilled nursing facility, inpatient rehabilitation or long term acute care hospital)
This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).

We have an Accredited Stroke and Chest Pain Center. Our hospital is a premier area destination for Women's & Children’s healthcare services. We offer the region’s only Maternal Fetal Health Center to care for critically ill mothers and their babies. Our hospital has an antepartum unit, a labor and delivery unit, and a Level III NICU.
