Job description
Position Title: Wellness Assistant
Classification: Non-Exempt / Full-Time
Location: 601 S. Queen Street, Lancaster
POSITION PURPOSE
The Wellness Assistant works closely with the Health Supervisor and Wellness Team to provide technical support that ensures all Thrive to Five children have their health and nutrition needs met, are up-to-date on all medical and dental preventative care, and have completed hearing, vision, nutrition, and dental screenings within the first 45 and 90 calendar days of entry into school.
DUTIES AND ESPONSIBILTIES
- -Works with Health Supervisor and Wellness Team members in coordinating sensory screenings within 45 day period/90 day period-dental, assisting in notifying staff and insuring families are notified of scheduled dates. Assists in monitoring to ensure screenings are completed within the required timelines
- -Assists in performing vision and/or hearing screenings in meeting program needs.
- -Collaborates with the Health Supervisor and the Coordinator of Family Supports to access and/or create resource materials for staff to use and share with children and families around health, safety, nutrition, mental health, and disabilities topics as needed
- -Works with the Health Supervisor to securely access children’s health information from health and dental providers for the purposes of compiling and completing each child’s medical record.
- -Works Collaboratively with the Health Supervisor, Early Learning Supervisors, and program staff to address individual health needs of children, inclusive of the tracking of referrals for health needs, the creation of organization tools, and data entry.
Function 2: Data and Record Keeping
- -Monitors, and reports on program compliance in delivering health services.
- -Maintains current health records of children. This includes filing and entering health records into database.
- -Monitors the accuracy of data entry of health information and tracking of referrals and services, following Head Start, PKC, and DHS guidelines; provides reports to the Health Supervisor.
Function 3: Staff Education/Training
- -Collaborates with the Health Supervisor and the Coordinator of Family Supports to access and/or create resource materials for staff around health, safety, nutrition, mental health, and disabilities topics as needed.
- -Assists the Health Coordinator in the planning of and the organization of needed resources for staff professional development around the topics of health, safety, nutrition, mental health, and disabilities.
Function 4: Nutrition
- -Monitors, and reports on program compliance in delivering nutrition services.
- -Works collaboratively with the Health Supervisor, Early Learning Supervisors, staff, and CAP/outside agencies to provide nutrition education to children and families.
- -Works collaboratively with the Health Supervisor to support the systems for tracking children with special dietary needs, inclusive of the tracking of special diet referrals, the creation of organizational tools, and data entry.
Function 5: Community Partnerships
- -Collaborates with outside agencies in working to meet children and families’ health needs in accordance with Head Start Performance Standards, PKC standards, and DHS standards.
- -Organizes meetings with community partners, inclusive of making contact, setting up dates and locations, attending the meetings, and maintaining documentation around the meeting and next steps.
- -Sends out periodic program updates to community partners around programming changes, strengths, and needs.

- -Observes CAP Values in working with children, families, staff, and community partners.
- -Follows all Community Action Program/Head Start policies, procedures and systems
- -Participates in self-assessment and collaborates to develop improvement plans
- -Participates in ongoing training to fulfill job responsibilities and Professional Development Plan developed at annual evaluation
- -Helps develop and participates in program pre-service, mandated and ongoing professional development identified by assessment, interest and needs
- -Attends necessary meetings
- -Maintains mutual confidence and trust with center teams.
QUALIFICATIONS/JOB FACTORS
Other:
- -Ability to speak, read and write fluently in English; effective communication skills, both orally and written
- -Ability to read and interpret documents such as performance standards, information memoranda and program guidance; ability to write reports and correspondence; ability to speak effectively before groups of clients, employees or community partners.
- -Ability to use data base software to enter family and program data and Microsoft Office programs.
- -Positive recommendation from previous employer/supervisor regarding work performance and relevant knowledge and skills.
- -PA Act 33 and 34 certifiable , FBI clearances
- -Current driver’s license and vehicle available to travel to Head Start sites and professional development.
- -Respect and sensitivity toward cultural differences.
- -Demonstrated success in working as a team member.
- -Reasonable close and distant visual acuity to conduct staff observations.

WORK ENVIRONMENT/CONDITIONS
- -Individual may work at the site they are visiting for the day.
- -Individual may start and end their day at home when they are visiting a site outside of where their assigned office space is.
- -When not visiting a site, individual should report to their assigned office space.

- Ability to work general hours of operation with some flexibility based on program need.
