We are seeking a passionate individual to provide short term case management services to youth and their families in their natural environment, schools, and various community settings.

Case Management Duties include:

● Provide short-term, community-based case management to assigned families following a behavioral health crisis

● Safety planning and increasing healthy coping skills

● Develop treatment plans

● Assist with accessing supports, resources, and outpatient services that will help stabilize the crisis and keep them safe in the community

● Function as part of an interdisciplinary team and develop and maintain effective relationship with community professionals

Case Management Qualifications:

● Bachelor’s degree or Master’s degree in behavioral health or related field

● Must be eligible for an NPI number and an Ohio Medicaid number

● Acceptable driving record

Preference given to the following candidates:

● Prior case management experience

● Understanding of child and adolescent development, behavioral health, and community resources

● Past or current experience in motivational interviewing, crisis intervention, and trauma informed care

This crucial position is available thanks to a nearly $4 million SAMHSA grant that Coleman received to implement Certified Community Behavioral Health Clinics (CCBHCs) in our service areas of greatest need.

Job Type: Full-time

Pay: $17.00 - $18.50 per hour
