Nursing Care Coordinator

Adaptive Home Health is looking for a nursing care coordinator to join our fast-growing team providing high quality care to patients throughout all of North Texas and Austin. Within this role you will have an opportunity to both strengthen your knowledge of the home health care industry and grow alongside a team of dedicated professionals.

Responsibilities:
Work with the clinical team to create patient schedule
Create patient profile's in our EMR (KanTime)
Coordinate with and assign clinician schedules
Field patient calls to address any scheduling concerns
Work with clinical team to ensure patients are seen in accordance with their plan of care
Communicate schedule changes between office staff and field staff
Participate in the on-call schedule

Requirements:
Successful background check
Drug Screen
Proficiency with Microsoft Office
Customer service experience
1-2 years experience (preferred)

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
