Care Coordinator

This position will monitor the treatment process including: intake, necessary referrals, educating, treatment plan reviews in order that clients have a full understanding of services offered and recommended, allowing for highest levels of involvement in their treatment. This position will work closely with all internal and many external service providers to assist in establishing care pathways that provide for all health needs of our clients.

Must have:

• Excellent written, verbal, and organizational skills.
• Ability to manage multiple tasks simultaneously.
• Relationship-building and motivational skills that help others achieve their goals.
• Ability to motivate and support clients with care, empathy and support.
• Good at interpreting data, measuring outcomes, problem-solving, and establishing person-centered motivational strategies.

Job Type: Full-time

Pay: From $25.07 per hour

• High school or equivalent (Required)

Experience:

• Customer service: 1 year (Preferred)
