A Community Health Worker (CHW) is a liaison and provides cultural mediation between the health care system, social services, and the community. A CHW is a trusted member, and has a close understanding of, the ethnicity, language, socio-economic status, and life experiences of the community served. A CHW assists people to gain access to needed services and builds individual, community and system capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, navigation and follow-up, community health education and information, social support, advocacy, and encouragement in participation of clinical screenings. Travel is approximately 75%

Key Responsibilities
  • Conduct social determinants screening to address areas of need for McKesson Employees
  • Collaborate with other McKesson Specialty Health departments/business units and US Oncology Network practices as well as external community-based organizations to provide support to employees
  • Following a defined protocol, conducts culturally appropriate skill building health literacy education sessions on different health topics for groups and individuals.
  • Become fully versed in McKesson Corporate Benefits
  • Administer evaluation tools to assess program’s effectiveness; Document plan, goals, and follow-up in a timely manner; Responsible for tracking work activities and results with close attention to detail.
  • Coordinate and encourage staff to get preventative screenings
  • Coordinate and participate in other projects and duties as needed or requested

Minimum Job Qualifications (Knowledge, Skills, & Abilities):

Education/Training – High school Diploma required. DSHS CHW Certification required.

Business Experience – Proficiency in Microsoft Office Products (Outlook, Excel, Word, Teams, and PowerPoint) required; Must have exceptional communication skills and MUST be able to speak to the Providers and Other Healthcare Professionals

Specialized Knowledge/Skills – Experience with community-based outreach required; Experience in health education and/or addressing health literacy required; Presentation skills required. Must be bilingual, English/Spanish proficiency.

Working Conditions:

Environment (Office, warehouse, etc.) –
  • Traditional office environment. Travel between two DCs in Chino, CA & Orlando, FL.

Physical Requirements (Lifting, standing, etc.) –
  • Based out of the distribution center, performs computer-based work, in-person work is required, travel will be ~75% of duties. Must be willing to travel to Chino, CA & Orlando, FL and surrounding areas.

Our Base Pay Range for this position
