Job Summary
Directs the ongoing development, implementation, and coordination of physician network nursing, clinical education and clinical support programs with the aim of enhancing the quality of patient care and outcomes in the physician network. Responsible for communicating and collaborating with physician network leadership, management, internal customers, and external vendors to ensure successful implementation and completion of projects, which are related to physician network clinical support initiatives. Outstanding leadership, communication, collaboration, and change management skills required.
Minimum Qualifications
Minimum Education: Master’s degree in Nursing, Healthcare Administration, or a Related Field
Minimum Years of Experience: 5 Years of experience in a healthcare environment/ setting;
3 Years of experience managing clinical education, performance improvement or clinical support programs or Healthcare IT/ Clinical Informatics program (Can be concurrent with experience listed above)

Required Certifications/Licensure: Licensed as a Registered Nurse
Required Training: Demonstrated experience with progressive project management and change management;
Requires an understanding of health care operations, physician office workflows, electronic health records (EHR), practice based quality initiatives, governmental regulatory programs, and third party payer programs;
Proficient in the use of MS Office Applications
Essential Functions
- Facilitates the provision of patient-centered care across the Physician Network through the use of best practices to identify and implement process and system improvements.
- Responsible for the growth, development, and oversight of the following:
a. PN clinical competency and assessment programs
b. PN clinical education programs including orientation and the MOT Residency Program
c. PN clinical policy and procedure development and ongoing updates
d. Practice-based safety rounds
e. Clinical support programs, including, but not limited to, diabetes education, chronic care management, Safeside Suicide prevention, tobacco cessation, Reach out and Read
f. Ambulatory transition of care management and improvement strategies
g. Clinical process improvement - Develops department(s) goals and objectives, and establishes and implements policies and procedures for department operation.
- Works closely with various committees and clinical specialists to identify opportunities for performance improvements related to patient care.
- Provides leadership for quality improvement and clinical staff development and works closely with other healthcare team members to facilitate quality care.
- Identify staff educational needs and implement measures to address needs.
- Provides formal and informal education for clinical staff.
- Through the use of clinical experts assists staff with meeting patient needs and improving care provided to patients.
- Will embody those behaviors, philosophy, and values which serve to uphold the ideals and enhance the image of Lexington Medical Center.
- Participates in Physician Network, interdisciplinary and nursing committees and promotes performance improvement initiatives.
- Shares knowledge with others and promotes growth and development of others.
- Participates in professional organizations as LMC representative.
- Responsible for own professional growth and development to keep abreast of evidence-based literature and regulatory standard updates.
Duties & Responsibilities
- Functions as a professional role model for other staff.

- Promotes research and data analysis.
- Assesses human resource needs and plan and evaluate employee activities to ensure safe, effective and fiscally sound operations.
a. Develops standards of performance for staff within the department, conducts performance appraisals and oversees disciplinary actions within the department.
b. Responsible for development and adherence of FTE budget.
c. Knowledgeable of HR processes and uses HR personnel appropriately.
d. Promotes continual learning environment which provides opportunities for the continued professional development and broadened clinical knowledge of staff. - Responsible for the development and adherence to operating and capital budget based on projected needs and constraints.
- Maintains up to date data and project status reports for physician network leadership.
- Works closely with the Risk Management Department to identify opportunities for improvements related to patient safety within the physician network.
- Participates, facilitates and/ or leads projects with other directors, managers and teams to plan, implement and/or optimize clinical systems that support the ambulatory care initiatives. Responsible for directing on time projects, expected deliverables, and resources for all clinical support initiatives.
- Works with multiple organization stakeholders, using critical thinking to triage issues, and continuously evaluate methods to improve all quality initiative reporting processes.
- Demonstrates leadership, collaboration, critical thinking, and teamwork in addressing decisions and resolving issues.
- Collaborates with multidisciplinary teams in the areas of administrative, operational, clinical, administrative, managed care, and finance.
- Serves as a champion and change manager for all assigned areas of responsibility.
- Performs all other duties as assigned.
