Job Summary
We are an Early Intervention agency providing service coordination, evaluations and services to children with developmental delays/disabilities in the 5 boroughs.
The Service Coordinator is the primary contact between service providers and parents. The Service Coordinator helps families understand their rights in the NYS Early Intervention program and available services to address the concerns they have for their children.
We have an immediate opening for a full-time Service Coordinator in Brooklyn, Queens, Manhattan, Bronx and Staten Island. Applicant must have current or previous experience with Service Coordination in NYC.

Job Title: Fee-for-Service Service Coordinator

Job Type: Full-time
This position includes a flexible schedule, as well as the option of working from home.
RESPONSIBILITIES
- Guide families through the early intervention process
- Provide excellence in case management services to young children with special needs and their families in community and office based settings
- Support families through the evaluation process and obtain proper services for eligible children
- Participates in the development, review and evaluation of individualized family service plans (IFSP).
- Advocate for children by monitoring the quality of services provided
- Attend home visits and meetings as scheduled
- Help families of children transition between services
- Ensure the implementation of all authorized services, no gaps in services
- Prepare and submit daily session notes and other paperwork timely and accurately.
- Provide services in accordance with Early Intervention Regulations.
QUALIFICATIONS
- 3 years prior experience in Service Coordination required
- Bachelor's degree in Social Services Education or related services (e.g. Psychology, Social Work, Education, Sociology)
- Strong knowledge of early intervention regulations
- Bilingual Spanish preferred
- Able to commute within the 5 boroughs via car or public transportation
