Responsibilities will include:
- Review the resident/patient evaluation with the referring Physical Therapist
- Provide treatment to the resident/patient in accordance with the goals and plan set by the referring Physical Therapist
- Provide accurate professional documentation as required by departmental policy and maintain departmental compliance with the state and federal regulatory guidelines
Requirements:

Associate degree from accredited P.T.A. educational program.

Minimum of 2-5 years experience in Long Term Care preferred.

Current New York State Physical Therapist Assistant license or eligibility.

Geriatric related experience preferred.
