Purpose of Position:

Provides person-centered, strength-based assessments, evaluating barriers to access and adherence that are tailored to develop treatment/care plans focused on the individual needs and goals of the patient. The assessments will integrate health care a social support service

Responsibilities:

• Develop an interdisciplinary, patient-centered care plan
• Ensure patients with multiple chronic conditions/ disabilities achieve the highest personal level of functioning
• Improve communication among providers/ care team, patients, and their caregivers
• Offer a range of services for high risk patients, including psychotherapy, and social support programs
• Support family caregivers to reduce stress and burdens

For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
