Hospital Manager
Job description
The Position:
St. Patrick Center is hiring case managers due to our agency expanding and so is our Hospital to Housing Program. We are looking for candidates with exceptional interpersonal skills for the position of case manager. The Hospital to Housing Case Manager will work in a community setting to engage homeless and/or housing insecure patients, identify and secure services for their physical, behavioral and basic needs outside of emergency medical services, and act as advocate and liaison, consulting with community agencies and providers to maintain coordination of care and transition, among other duties. Moreover, the Hospital to Housing Case Manager will have an understanding of mental illness and substance use. This position works Monday-Friday, 8:00 AM-4:30 PM.
